Application and Medical Forms

Complete and mail the following two forms:

  1. Application Form including:
    • White Stag Leadership Development Registration Form
    • Parent Permission and Consent for Medical Treatment
    • Health and Medical History Record
    • (A standard BSA medical form may be used instead.)
    • BSA Youth Membership Registration (signed by youth and parent)
  2. Medical Examination Record included in the Application Form or theBSA Personal Medical Record (signed by a doctor).

Enrollment is Limited

Application Form and camp fees are accepted on a first-come, first-served basis. Your registration will be dropped and no refund will be made if we do not receive all required forms by the deadline.

Special Medical Needs

Youth with minor medical needs, including ADHD and related challenges, are welcome in camp. All prescription medications must be brought to camp and dispensed by our certified EMT and licensed nursing staff. Our camp cannot accommodate disabled or youth with serious medical needs. For more information, contact the Program Director.

Bring to Camp

All group equipment is supplied. Each youth must bring their own personal equipment.

Discounts

Sibling discount of $50 each is available for two or more sibling.
Group discounts of 10% are available for five or more participants.

To obtain a discount, indicate the sibling's name and / or unit number on each camper’s Registration Form.
