Complete the following two forms:

1. Application Form including:
• White Stag Leadership Development Registration Form
• Parent Permission and Consent for Medical Treatment
• Health and Medical History Record
• Medical Examination Record
• Standard Photo Release
2. Personal Medical Record. Required for each youth.

Early enrollment discounts end on May 1. To secure a place at camp, fax the application form and medical record to (831) 624-0989 and then mail it, with your camp fees to:

Bring to Camp

All group equipment is supplied. Each youth must bring their own personal equipment.

Discounts

Sibling discount of $50 each is available for two or more sibling.
Group discounts of 10% are available for five or more participants.

To obtain a discount, indicate the sibling's name and / or unit number on each camper’s Registration Form.
